Types of Service
Every Connexion customer must be certified as eligible under either the guidelines of the ADA or TD program.
ADA Services
The Connexion ADA Eligibility process is based on the Easterseals Project Action model. It consists of an in-person interview and possible functional assessment: either mobility or cognitive. All clients, both new and recertifying, must complete this process to receive or continue to receive service. Learn more and apply for ADA eligibility.
TD Services
Connexion trips beyond the three-quarter-mile boundary of JTA fixed-route service may qualify for the TD program. The Duval County Transportation Disadvantaged Coordinating Board has established an eligibility process for the provision of non-sponsored service to Duval County residents. TDLS Services
The Temporary Disability Life Service (TDLS) provides six-month temporary transportation approval for oncology and dialysis patients who need assistance. To apply, individuals must complete an official application, which may include medical documentation. This service helps ensure access to vital health care treatments by offering paratransit or specialized transportation. To apply for the service please complete the TD eligibility application.

Frequently Asked Questions (FAQ)
The Americans with Disabilities Act of 1990 (ADA) requires JTA to implement a process that strictly limits eligibility to individuals who clearly meet ADA eligibility standards. The new in-person assessment process will identify individuals' potentials rather than limitations in their ability to use accessible bus transit.
You may be required to undergo a functional or cognitive assessment as determined by your disability. The results, coupled with your travel habits, will determine the best way to serve your transportation needs.
The assessment will be conducted by either a Physical or Occupational Therapist.
There is no charge to you for the assessment.
The assessment will be conducted at the Connexion Eligibility and Training Center located at 100 North Myrtle Avenue, Building 2, Jacksonville, FL 32204.
A brief interview will be conducted by JTA staff to review your application, your photo will be taken, and then, if necessary, you will meet with the Physical or Occupational Therapist for either a mobility or cognitive assessment.
The results of your assessment will go through an internal review process and within 21 days, you will be notified of your eligibility status.
An appeal process is available to any applicant who is denied eligibility.
